Authorities identify victim of wind turbine accident as 34-year-old Goldendale, Wash., man

Published: Monday, August 27, 2007
Noelle Crombie, The Oregonian

Sherman County authorities have identified the victim of Saturday's wind turbine accident as Chadd B. Mitchell, a 34-year-old Goldendale, Wash., man.

Sheriff Brad Lohrey said Mitchell worked for the turbine's German manufacturer, Siemens. Winds at the time of the accident were about 25 mph, Lohrey said.

Mitchell died when a wind turbine on the not-yet-opened Klondike III wind farm east of the town of Wasco snapped in half. A second worker, Bill Trossen, of Minnesota, was inside the 242-foot-tall shaft. He was taken to an area hospital and was later released. Trossen's age and hometown were not available Monday morning.

The turbine broke about 4 p.m. Saturday a little more than halfway up the nearly hollow tube that holds the blades, said Geremy Shull, a Sherman County sheriff's deputy. The men were maintenance technicians, he said.

It was the first death associated with a wind turbine collapse in Oregon. While fire, ice and storms can damage turbines, they rarely fall over.
Last year, a wind gust bent and twisted a similar tower at the Condon Wind Project, a joint venture in Gilliam County between SeaWest Windpower of San Diego and Bonneville Power Administration, in operation since 2002. No one was injured in that case.

Hundreds of the turbines are going up in wheat fields and other windy areas of Oregon and Washington. This year alone, Oregon developers are on track to more than double the wind power generated by turbines in the Columbia River Gorge. The construction boom is expected to continue through 2008 or longer if Congress extends a federal tax credit.

Siemens fined $10,500 for wind turbine tower collapse that killed worker
Published: Tuesday, February 26, 2008
Shelby Wood, The Oregonian

The state has fined Siemens Power Generation $10,500 for safety violations related to the Aug. 25 collapse of a wind turbine tower in Sherman County that killed one worker and injured another.

The 6-month investigation by the Oregon Occupational Safety and Health Division found that Siemens failed to properly instruct and supervise workers in the safe operation of tools and equipment. It also found that company procedures for working under potentially dangerous conditions fell short of OSHA regulations.

The investigation did not find any structural problems with the tower itself.
"The tragedy was the result of a system that allowed the operator to restart the turbine after service while the blades were locked in a hazardous position," said Michael Wood, OSHA administrator.

The event occurred at the Klondike III wind farm near Wasco where three workers were performing maintenance on one of the turbine towers. Turbine blades positioned flat to the wind were released before other measures were followed, creating an "overspeed" condition that caused one of the blades to strike the tower. The tower then buckled and collapsed.

Chadd Mitchell, who was working at the top of the tower, died in the collapse. William Trossen, who was on his way down a ladder inside the turbine, was injured. The third worker was outside the tower and was unharmed.

Oregon OSHA, a division of the Department of Consumer & Business Services, enforces the state's workplace safety and health rules.

Parents of technician killed in Sherman County wind-turbine collapse file suit
Saturday, February 06, 2010
Aimee Green, The Oregonian

The parents of a 34-year-old technician who died when the 230-foot wind-turbine tower he was in collapsed to the ground in a Sherman County wheat field is suing for $7 million. In their lawsuit, Gail Eikanas and Jerry Mitchell fault the Danish turbine maker Siemens Wind Power A/S, the Klondike Wind Farms III and owners PPM Energy, among others. Their son, Chadd Mitchell, died in August 2007. He was the father of two.

The suit was filed Thursday in Multnomah County Circuit Court. According to the suit, the wind turbine's rotor went into "overspeed," causing the structure to crash to the ground and crush Mitchell. In early 2008, the Oregon Occupational Safety and Health Division fined Siemens Power Generation $10,500 for safety violations, after a six-month investigation. The agency found that Siemens didn't properly train or supervise the employees on the job, and that

Mitchell and another worker had less than two months of experience and were working without a supervisor. Siemens disagreed with the findings of safety violations, and appealed the findings. It's not clear what the outcome of that appeal was.

Burbank High graduate killed in accident

Charles Walter "Walt" Rosenthal, BHS 1965
April 29, 2006, Chris Wiebe, Burbank Leader
Friends and family remember Charles "Walt" Rosenthal as a sincere and kind-hearted man who had a passion for the outdoors and who was to turn 59 on Wednesday.
But tragedy brought that passion to an end April 6 when Rosenthal, a Burbank High School graduate, died in an accident at Mammoth Mountain near China Bowl run.
As a member of the Mammoth Ski Patrol, Rosenthal and two fellow patrolmen were surveying a hazardous snow area when the two slipped into a volcanic gas vent. Rosenthal fell to his death in an attempt to rescue the two men, said Dep. John Estridge of the Mono County Sheriff's Department. The two men, James Juarez, 35, and John "Scott" McAndrews, 37, died.
Rosenthal was a researcher for UC Santa Barbara's Institute for Computational Earth System Science, working in the field in Mammoth. During the snow season, he served as a member of the Mammoth Ski Patrol. After graduating from Burbank High in 1965, Rosenthal earned a bachelor's degree from UCLA, then went on to obtain a master's degree in geography at UC Santa Barbara, according to a news release from UC Santa Barbara.
The focus of his professional research was snow hydrology, how snow grains bond together for a stable snow pack. Rosenthal had been awarded a National Science Foundation grant to fund his studies and intended to continue doctoral studies in Santa Barbara in the fall. He had been living at Mammoth Lakes.
Rosenthal is survived by his wife, Lori Michelon, and his 14-year-old daughter, Lily.

UC Santa Barbara Researcher Dies in Mammoth Mountain Gas Vent Accident
Santa Barbara, Calif.
A UC Santa Barbara researcher was among three men who died at Mammoth Mountain in the eastern Sierra on Thursday, April 6, when the trio, all members of the local ski patrol, fell into a volcanic gas vent as they tried to rope off the unsafe area.
Charles Walter Rosenthal, 58, was an assistant specialist with UCSB's Institute for Computational Earth System Science. He was based at a field site in Mammoth known as the Sierra Nevada Aquatic Research Laboratory (SNARL). It is a component of the Valentine Eastern Sierra Reserve, which is a part of the UC Natural Reserve System. He also was employed seasonally as a member of the ski patrol at Mammoth Mountain. He reportedly died when he tried to rescue two colleagues who first fell into the hole, called a fumarole, near the bottom of the China Bowl run.
Rosenthal, who was known by his middle name, had worked as a researcher for the Institute for Computational Earth System Science (ICESS) since 1991. He earlier had been a teaching assistant in the Department of Geography, where he earned a master's degree. He was expecting to return to UCSB in the fall to continue work on his Ph.D. He resided in Mammoth with his wife, Lori, a printmaker in the eastern Sierra Nevada, and their 14-year-old daughter, Lily.
In a statement, UCSB Chancellor Henry T. Yang said: "Our UCSB family is deeply saddened by the tragic loss of our colleague Walter Rosenthal. He was a researcher doing important work in the field of snow hydrology. He was a highly respected guide and mentor for many, known for his spirit in helping others. Our hearts go out to his family and loved ones, who have suffered such a terrible loss. Our campus community will miss Walter, and will always remember and appreciate his many contributions. Our campus flag is lowered today in his memory."
Rosenthal was involved in snow hydrology research and remote sensing. He worked closely with geographer Jeff Dozier, the founding dean and a professor at UCSB's Bren School of Environmental Science and Management.
Said Dozier: "As our friend, colleague, guide, and teacher, Walter had an insightful intellect and an engaging enthusiasm for the study of snow. He had worked in the ski area since the early 1980s, but had always maintained an interest in research and had returned to the university in his 40s to complete his master's degree in geography in 1994."
At the time of his death, Rosenthal and Dozier had been awarded a grant from the National Science Foundation to examine sintering in snow, the process by which snow grains bond with each other and increase the stability of the snow pack.
"Having realized that four decades of sintering theory were at odds with observations and with theories for other materials, Walter was planning to enroll in the Bren School's Ph.D. program next fall," said Dozier. "His work would have changed the way the world thinks about processes within the mountain snow pack."
Daniel Dawson, the director of the Valentine Eastern Sierra Reserve of the UC Natural Reserve System, called Rosenthal "a gifted mathematician who did novel work in image processing." He noted how Rosenthal had been a ski patrol member for many years and had served as the lead avalanche forecaster for Mammoth Mountain. "Supposedly, April 1 was his last day at the mountain and he just went back in yesterday to help out, knowing they would be shorthanded after the big storm."
A memorial service for all three ski patrol members who died--James Juarez, Scott McAndrews, and Rosenthal--will be held Friday, April 14, at 6:30 p.m. at the old gondola station, Main Lodge, Mammoth Mountain. The memorial is open to the public. A memorial fund to benefit Rosenthal's family has been established at the Union Bank of Mammoth Lakes. Contributions to the Walter Rosenthal Memorial Fund can be sent to Union Bank, PO Box 2729, Mammoth Lakes, CA 93546.

Alan Lloyd Buckley, BHS 1966

Alan Lloyd Buckley passed away in the Metrolink train crash in Chatsworth, California on September 12, 2008. Alan Lloyd Buckley, 59, of Simi Valley, was a mechanic for the city of Burbank
All day Saturday, the death toll from Friday's Metrolink crashed crept up, from 17 in the morning to 25 by the end of the day. All day, one family after another found out that their loved ones did not survive.
One of those families was the Buckleys, of Moorpark. Alan Buckley, 59, died at the scene in Chatsworth, but his family didn't find out until early Saturday morning, said his son, Jeff Buckley.
Alan Buckley was the son of a Union Pacific railroad worker and a train buff since childhood, his son said. He was on board the day the service opened in 1992, and just about every day since then, he took the train to his job in Burbank, where he was a mechanic for the city.
"Metrolink was his thing," Jeff Buckley said. "He knew the conductors, he rode up front with them. He even took the train on weekends. ... For him to pass away this way is irony, I guess. He just loved trains."
Buckley, who would have turned 60 next week, was a man of long-term commitments. He worked for the city of Burbank for 32 years and was married for 38 years.
He leaves two children and five grandchildren.
